This initial intake form is the first step in registering your child for Ski Club. The last day to complete the form is October 30th.
Additional paperwork will be distributed to students during the first week of November.
If more than 50 students sign up, a waitlist will be created. Preference will be given to eighth-grade students.
If filling out for multiple children
please fill out a new form for each child.
