There has been understandable confusion regarding the 5th grade field trip forms that were recently sent home.
Fifth grade will be taking a trip to Sturbridge village sometime in June. Before we can finalize the date of the trip we need to find out if there needs to be a nurse on the trip, hence the health portion of the form. Once we know if we need a nurse we can determine the availability of a nurse and, based on that, we can set a date.
There will be another sheet, once details are finalized, with all the relevant information.
Please get the health forms in as soon as possible so we can schedule this exciting trip.
